Vital Pulp Therapy

Vital Pulp Therapy (VPT) is a group of minimally invasive endodontic procedures designed to preserve and maintain the health of the dental pulp when it has been compromised by deep tooth decay, trauma, or restorative dental work. Instead of completely extracting the nerve tissue (as done in standard root canal treatment), VPT removes only the damaged or infected portions. This allows the remaining healthy pulp to survive, keeping the tooth naturally vital. ’More

Main Types of Vital Pulp Therapy 🔗

The choice of procedure depends directly on the depth of the cavity and whether the inner pulp chamber has been completely breached: 

  • Indirect Pulp Capping (IPC): Used when decay is very close to the pulp but hasn't fully exposed it. A protective sedative material is placed over a ultra-thin layer of remaining dentin to promote healing and block bacteria. 

  • Direct Pulp Capping (DPC): Performed when a small portion of healthy pulp becomes exposed during cavity removal or trauma. The dentist places a specialized biocompatible material directly over the exposed nerve. 

  • Partial Pulpotomy (Cvek Pulpotomy): Usually used for traumatic injuries or small bacterial exposures. The doctor removes only a few millimeters of the inflamed pulp tissue just beneath the exposure site before sealing it. 

  • Complete (Coronal) Pulpotomy: Performed when inflammation has spread throughout the entire upper pulp chamber. The entire coronal pulp is removed down to the root canal openings, leaving only the healthy root canal nerves intact. 

 

Advanced Materials Used

Modern VPT success relies heavily on biocompatible materials that stimulate the pulp to regenerate and build a protective layer of new dentin (a dentinal bridge). Commonly used agents include: 

  • Mineral Trioxide Aggregate (MTA): A highly effective calcium silicate cement known for its excellent sealing ability and tissue compatibility. 

  • Biodentine / Bioceramics: Advanced pure silicate materials that offer fast setting times, lower risk of tooth discoloration, and superior healing properties. 

  • Calcium Hydroxide: A traditional clinical material used for decades to stimulate secondary dentin formation. 

 

Indications and High Success Rates

According to position statements from organizations like the American Association of Endodontists (AAE), VPT is highly recommended for young permanent teeth with incomplete root development to ensure the roots finish growing (apexogenesis). However, it is also highly successful in mature adult teeth: 

  • Eligible Cases: Teeth diagnosed with normal pulps or reversible pulpitis (mild inflammation causing short, sharp pain from hot or cold triggers). It is also increasingly used as a conservative option for select cases of asymptomatic or symptomatic irreversible pulpitis, provided bleeding can be controlled during the procedure. 

  • Success Rates: Clinical trials and systematic reviews demonstrate that when using modern bioceramics and proper dental sealing techniques, VPT boasts success rates ranging between 80% and 95%, proving comparable to conventional root canals in properly selected cases. 

  • Non-Eligible Cases: VPT cannot be performed on teeth with necrotic (dead) pulps, dental abscesses, or severe horizontal root fractures. These require full root canal therapy or extraction.
