Endodontic surgery encompasses a variety of specialized microsurgical procedures performed by endodontists to save a natural tooth when standard, nonsurgical root canal therapy is insufficient or cannot be performed. Modern techniques rely heavily on advanced tools like digital imaging, cone-beam computed tomography (CBCT), and operating microscopes to clean and seal the deepest parts of a tooth's root structure with high precision. What to Expect During and After

The ProcedureAn incision is made in the gum to expose the bone. Ultrasonic instruments or Piezotome devices are used for precise, gentle bone cutting. The root tip is treated, sealed, and the gum is closed with sutures.
Immediate RecoveryMinor swelling, bruising, and mild discomfort are normal. Most patients can drive themselves home and return to work or routine activities by the very next day.
Suture RemovalIf high-precision monofilament sutures are used during microsurgery, they are typically removed within 48 to 72 hours for clean, scar-free healing.
Long-Term HealingOver the course of several months, your jawbone will naturally heal and regenerate around the treated root tip.
Alternatives
The primary alternative to endodontic surgery is extracting the tooth entirely.
